FAQs
Health Clearance Form
Prior to or on arrival for your appointment you will be provided with a Health Clearance form. Please complete this in as much detail as possible prior to your appointment and email back to hello@theexercisetherapist.com.au
Cancellations / Rescheduling
If you need to cancel your appointment, you can do so through by calling (08) 9385 1430.
Missed appointments and late cancellations disadvantage therapists and other patients. If less than 24 hours’ notice is given for a missed appointment, you may be charged 50% of the cost of the consultation. Failure to attend will incur full consultation cost. We reserve the right to charge a cancellation fee, equivalent to the fee of the original scheduled consultation for repeat offenses.
